Aging In Place

Help your loved one stay at home for as long as possible. We understand that staying in your own home for as long as possible — often called Aging in Place — has its financial, medical and emotional advantages. Aegis Care Advisors will conduct an in-home assessment to examine all the ways that your loved one’s living situation and home environment can be adjusted, modified or strengthened to optimize safety, health and wellness, and avoid future injury.

Home life preservation services may include the following:

• Implementing new, post-op or chronic care plans
• Coordination of home-based services (PT, OT, Speech, Skilled Nursing, In-Home Aide)
• Ensuring RN wellness check-ins
• Attending and advocating at medical appointments
• Monitoring and coordinating medications with doctors and pharmacies
• Medical Alert Device evaluation and implementation
• Managing structural and design modifications to the home
